Hi Everyone,
Missed your chance to order an SCSF Club jacket this summer? Can’t choose between styles and want one of each? Now is the time to place your order.

We will be submitting a new order for SCSF Club jackets as soon as we get prepaid orders for 24 jackets. This is the minimum order needed to get the jackets with the club embroidery.

Here’s what you need to do:
1. Print out this page.
2. Fill in the form below.
3. Make out a check or money order to “SCSF” for the cost of your jacket(s).
4. Place your completed form and payment together in the SCSF Club box, or mail them to:
    P.O. Box 19120
    San Francisco, CA 94119-1205

NOTE 1: If you are ordering more than one jacket, you need to complete a separate order form for each jacket. However, you can just send one check/money order that covers the total payment for all jackets ordered.

NOTE: 2: Orders will not be processed if payment has not been received. SCSF will not cash your check/money order until we have received enough jacket orders to submit the club order.

Standard embroidery for all Club jackets is as follows. The front left chest area will have a small, embroidered club logo with yellow star outline, royal blue “SCSF,” and silver blade. Your name (or initials if you prefer) will be embroidered in yellow and will appear below the right pocket hem. The back of the jacket will be embroidered with “The Skating Club of San Francisco” in yellow bold lettering.
